• Please fill out all REQUIRED digital forms on patient portal PRIOR to your arrival at Hela in order to be seen. These will be sent to you via email the evening prior to your appointment. Or you can go to this link: https://helamedical.myaestheticrecord.com/login to fill in the forms once they have been emailed to you. Please note: you cannot do online bookings from this site (even though there is a link). We use a different system for booking. This system is ONLY for digital forms at this point. Click here to see what the portal page looks like. NOTE: On digital forms, if you answer YES to any questions, you must write something in the blank below for the system to accept/save your form. Be sure to SAVE your answers on digital forms. Thank you!

• Please arrive with minimal personal belongings (i.e. no large bags and no heavy outerwear)

• Please keep your mask on until your provider gives you permission to take it off.

• When inside the spa please maintain a safe social distance of at least 6 ft away from front desk staff member and anyone else who happens to be in the lobby.

• No food or drink in our medical office. We recommend you hydrate yourself / drink water prior to arrival.

• If you happen to get sick within 14 days AFTER your appointment please email or text us as a courtesy because some of our staff members live with vulnerable population.

Failure to comply with the above instructions will result in not being treated at your appointment and we will reschedule you to the next available appointment date.
